GOOD FAITH ESTIMATE

NO SURPRISES ACT

As of January 1, 2022, the government enacted the No Surprises Act, which requires all healthcare providers to supply their clients with a Good Faith Estimate (GFE) for the costs of their care. All Points North Counseling Services will provide the GFE upon scheduling and when requested. This GFE does not include unexpected expenses that could arise during treatment. The GFE is intended to provide uninsured or self-pay individuals with a clear and reasonable estimate of the cost of our services, so there are no surprises when it comes to your healthcare needs for an item or service, a diagnosis, and a reason for mental health services. The estimate is based on information known at the time the estimate was created.

If this happens, federal law allows you to dispute (appeal) the bill if you and your provider have not previously discussed the change and have not supplied an updated GFE.

The GFE does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. Your provider will supply you with a new GFE should this happen. Section 2799B-6 of the Public Health Service Act (PHSA) requires healthcare providers and healthcare facilities to inform individuals who are not enrolled in a plan or coverage or a Federal healthcare program or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request, or at the time of scheduling health care items and services to receive a GFE of expected charges. 

Note: The PHSA and GFE do not currently apply to individuals using insurance benefits, including "out of network benefits" (i.e., submitting superbills to insurance for reimbursement).

Timeline Requirements

 

Providers must supply a GFE of expected charges for a scheduled or requested service, including items or services reasonably expected to be delivered in conjunction with such scheduled or requested item or service. That estimate must be provided within the following associated time frames: 

  • No later than one business day after scheduling if the service is scheduled at least three business days before the appointment date;

  • No later than three business days after the date of scheduling if the service is scheduled at least 10 business days before the appointment date; or,

  • No later than three business days after the request if the uninsured or self-pay individual asks for a GFE (without scheduling the service). A new GFE must be provided within the specified time frames if the individual reschedules the requested item or service.

  • 90791: Initial Psychotherapy intake

  • 90837: Ongoing therapy appointments (approx.. 53-60 minutes)

  • 90847: Family/Couples appointments (approx.. 45-60 minutes)

  • 90853: Group Psychotherapy (approx. 53-60 minutes)

  • 90833: Psychotherapy Add-On to Psychiatric Appointments (included in a 30 minute appointment)

​Within your GFE, you will find one or more diagnoses. Below is a list of some common diagnosis codes used at our practice. Please note diagnosis may change due to client-specific reasons.  

  • Adjustment Disorder (F43.23)

  • Mental Disorder, Not Otherwise Specified (F99)

  • Major Depressive Disorder (F32.9)

  • Bipolar Disorder (F31.9)

  • Posttraumatic Stress Disorder (F43.10)

  • Generalized Anxiety Disorder (F41.10)

  • Obsessive Compulsive Disorder (F42.9)

  • Personality Disorder, Unspecified (F60.9)

At All Points North Counseling Services, our team understands the unique challenges you may face on your journey to better mental health. We take a personalized approach to counseling, tailoring our services to your specific needs to ensure we provide the best possible care. How long you need to engage in mental health services and how often you attend sessions will be influenced by many factors, including, but not limited to, your schedule, provider availability, life challenges, and personal finances.

You and your provider will continually assess the appropriate frequency of services and will work with you to determine when you have met your goals and are ready for discharge, and/or a new "Good Faith Estimate" will be issued should your frequency or needs change.
